Fitting Your Home to You

Community Housing Solutions’ Aging in Place Program (APP) provides low-cost, high impact home modifications to older adults, people with mobility issues, and other disabilities to improve accessibility, reduce falls, increase independence, and facilitate aging-in place. APP engages the expertise of an occupational therapist and a Community Housing Solutions staff member, who is skilled at home, repairs, and is a Certified Aging in Place Specialist.

The Aging in Place program includes a person-centered review of your home, prevention strategies to keep you safe, as well as low cost accessibility modifications that address changing mobility needs, enable independence, and prevent injuries and further health declines. This program can often be an expansion of another repair done to your home through our other home repair programs.

Eligibility and Items:

Eligibile Homeowners:

  • Must own their home within the City of Cleveland
  • Be 60+ years or older and/or have a disability
  • Must own and occupy the home
  • Must have lived in the home for at least a year and the home is not for sale
  • Current on their city taxes or on a payment plan
  • Fall within the appropriate income guidelines with a 50% of area median income or less

Types of Items Often Used to Keep You Safe:

  • Lifted toilet seats or raised toilets
  • Grab bars in around your home, in addition to the bathroom, such as in the garage, outside your home, in bedroom.
  • Interior and exterior railings.
  • Upgraded inside and outside lighting for better mobility in the dark
  • Shower upgrades, including shower seats, slip pads, hand held showers heads
  • Hands free faucets or raised sinks
  • Sinks with no under cabinet
  • Furniture removal/rearrangement
  • Transition strips and level floors
  • Door levers instead of knobs
  • Accessibility to mail box, phone, food, bathroom supplies
  • Ability to lock doors
  • Safe walkways on exterior and interior of home
  • Items to help activities of daily living easier.
